Township of Hampden ("Claimant") files this Municipal Claim against George V.
Shoemaker ("Owner") and all that certain tract of land including improvements thereon, owned
by the Owner and described as follows:

for unpaid sewer and/or trash services rendered from April 1, 2006 to April 1, 2007. Said
services were rendered and charged pursuant to Hampden Township Code of Ordinances
Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended.
Attached hereto is a statement of the kind and character of the services so rendered and
the prices charged therefor, said statement being a true and correct copy of the Owner's account
with the Claimant, which sum of $561.75 was duly assessed against the above Owner and
premises beginning April 1, 2006 and for which sum, which includes interest, costs and
penalties and in addition thereto attorney's fees pursuant to Hampden Township Code
Ordinances Chapter 18, Part 2, Section 209.6 and Chapter 20, Part 1, Section 106.4, as amended,
a lien is claimed against the above premises in accordance with the Municipal Authorities Act of
