The Petitioner(s), Jonathan Christopher Amati, requests that this Court order a change of name for
himself/herself and in support thereof represents as follows:
Petitioner(s) is an adult individual, whose date of birth is January 3, 1980.
2. Petitioner presently resides at 4902 Carlisle Pike, PMB 201, Mechanicsburg, Pennsylvania, 17050,
in the county of Cumberland.
3. Petitioner(s) desires this change of name for the following reason(s):
Petitioner seeks to retain his father's surname (Haug). Petitioner respectfully requests an accelerated
hearing prior to his USMC active duty assignment (tentatively June 14, 2007).
4. Petitioner is a citizen of the United States of America and his/her social security
number is 191-60-8275.
5. There are no judgments or decrees pending against the petitioner.
6. WHEREFORE, Petitioner respectfully requests that this Court enter an Order
changing the name of Petitioner from Jonathan Christopher Amati to
Jonathan Christopher Haug
