GUARDIAN'S COMMENTS:
Please note that we carry the prepaid hurial arrangements as an informatIonal item only. The funds
were paid to Malpezzi Funeral Home. Sharon DeBuek, Mrs. Dugan's daughter, assisted in the
personal information and is aware of the arrangements.
A claim has been filed through the Veterans Affairs for a widow's monthly payment. As of this
report date we are waiting for the results of that claim.
Our goal is to try to keep Mrs. Dugan living at the Bridges in the assisted care unit as long a
possible. She appears to be happy and very content there.
I verify that the foregoing infonnation is correct to the best of my knowledge, infonnation and belief;
and that this verification is subject to the penalties of 18 Pa. CSA. ~4904 relative to unsworn
falsification to authorities.
