Your petitioner, who is 18 years of age or older applies for letters of administration c.t.a., on the
estate of the above decedent.
Decedent was domiciled at death in Cumberland County, Pennsylvania, with his last family or
principal residence at West Shore Health & Rehabilitation Center, 770 Poplar Church Road, Camp Hill,
Pennsylvania 17011.
Decedent, then 78 years of age, died April 14, 2007, at West Shore Health & Rehabilitation
Center, 770 Poplar Church Road, Camp Hill, Pennsylvania 17011.
Decedent at death owned property with estimated values as follows:

Petitioner after a proper search has ascertained that decedent left a Last Will and Testament in
which he designated his brother, Dallas J. Paxton, as executor. The said Dallas J. Paxton has renounced
the right to administer the estate in favor of his son, Ricky 1" Paxton, Petitioner herein.
THEREFORE, petitioner respectfully requests the grant of letters of administration in the
appropriate form to the undersigned.

I, CHARLES E. PAXTON, of the Borough of Carlisle, Cumberlan
County, Pennsylvania, declare this to be my last will and testament
and revoke all wills which I have previously made.
I I give, devise and bequeath my entire estate, real
and personal, to my mother, Ruth P. Paxton, if living, otherwise in
equal shares to my two brothers, Dallas J. Paxton and Robert L. Paxton,
if living, and if either shall be deceased to his surviving issue per
stirpes.
II I appoint my brother, Dallas J. Paxton, as executor
of this will. If for any reason he shall fail to qualify or cease to
act as such during the administration of my estate, I appoint Farmers
Trust Company of Carlisle, Pennsylvania, as substituted executor. I
direct that no bond shall be required of any fiduciary named in this
will.
IN WITNESS WHEREOF, I have hereunto set my hand and seal
this 7th day of December, 1982.
