The petition of the undersigned respectfully represents that:
Your petitioner, who is 18 years of age or older applies for letters of administration on the estate
of the above decedent.
Decedent was domiciled at death in Cumberland County, Pennsylvania, with his last family or
principal residence at West Shore Health & Rehabilitation Center, 770 Poplar Church Road, Camp Hill,
Pennsylvania 170 II.
Decedent, then 78 years of age, died April 14, 2007, at West Shore Health & Rehabilitation
Center, 770 Poplar Church Road, Camp Hill, Pennsylvania 17011.
Decedent at death owned property with estimated values as follows:

Petitioner after a proper search has ascertained that decedent left a Last Will and Testament as to
which probate has been refused in the discretion of the Register of Wills for lack of an oath of a
subscribing witness. The sole heirs-at-Iaw of the decedent are his brother, Dallas J. Paxton, of 8
Kenwood Avenue, Carlisle, PA 17013, and his brother, Robert L. Paxton, of916 Colver Street,
Muscatine, IA 52761. The said Dallas J. Paxton has renounced the right to administer the estate in favor
of his son, Ricky L. Paxton, Petitioner herein.
THEREFORE, petitioner respectfully requests the grant of letters of administration in the
appropriate form to the undersigned.
