PETITION FOR SETTLEMENT OF SMALL ESTATE
1. Your petitioner is Frances Mary Sabadish, who is the surviving
spouse and administrator of the estate of John Joseph Sabadish. 0

for almost 55 years prior to his death.

3. John Joseph Sabadish died intestate on the 6th day of~ril, 20(tf,

at 2:20 o'clock a.m., and at the time of death the decedent's principal residente
was 621 Wayne Drive, Mechanicsburg, Cumberland County, Pennsylvania.
4. The above-named decedent died intestate and no letters of
administration have been issued.
5. The names, addresses, and relationships of all persons having an
interest in the estate of the decedent as heirs or next of kin are as follows:

6. The decedent was survived by Frances Mary Sabadish, who is the
decedent's surviving spouse, and is entitled to the family exemption under 20 Pa.
C.S.A. Section 3121.
7. The total value of the decedent's personal estate is less than
$25,000 and consists of the following assets which have the following values:

8. The expenses of the decedent's last illness and funeral have been
paid, and there are no unpaid creditors of the decedent.
9. Because the sole heir is the surviving spouse, and the spousal
Inheritance Tax rate is 0.00%, there is no Inheritance Tax due to the
Commonwealth of Pennsylvania.
10. It is proposed that the following distribution of the decedent's
personal estate be made to the following heirs or next of kin.

WHEREFORE, the petitioner respectfully requests this Honorable Court to
order the PA Central Federal Credit Union to distribute the value of the
decedent's account no. 82809-015 to the surviving spouse who is entitle thereto
