You are ordered to active duty as a member of your Reserve Component Unit for
the period indicated unless sooner released or unless extended. Proceed from
your current location in sufficient time to report by the date specified.
You enter active duty upon reporting to unit home station.
REPORT TO HOME STATION: 24 October 2004 , Harrisburg, PA
REPORT TO MOB STATION: 27 October 2004 , Fort Lewis, WA
Period of active duty: Not to exceed 545 days
Purpose: OPERATION ENDURING FREEDOM (KFOR)

Additional instructions: .
(a) Early reporting is not authorized.
(b) You are ordered to AD with the consent of the Governor or other appropriate
authority of your state.
(c) Sure Pay is mandatory. Bring the appropriate documentation to authorize
sure pay to the bank (SF 1199). Bring copies of marriage certificate,
divorce decree, birth certificate of natural children or documentation of
dependency/child support, family care plan, wills, powers of attorney and
other documents affecting pay status. Pay status reported in DJMS-RC A24.
Transaction must be 0 (Alpha Character). Per Diem Authorized lAW JFTR.
(d) This is an unaccompained tour. Movement of HHG and dependents not
authorized. Non temporary storage of HHG is authorized. Storage of
one POV is authorized.
(e) 10 Card and Tags required on person while on Government orders.
Panographic Dental X-Rays or SF 603 and HIV Screen required if not current.
You and your dependen~s, as your agents, authorized PX/COM/THEATER and
med care during the period covered by this order.
(f) Send DO Form 214 to Unit of Assignment. You will return to the place of
initial entry on AD for Outprocessing/Refrad.
(g) Demobilization of units is prohibited without prior approval of HQ DA.
Individual members of the unit will demobilize with the unit unless
provisions of AR 635-100 or AR 635-200 apply.
(h) The National Defense Authorization Act 2004 see 703 authorizes early
eligibility for health care benefits.
(i) A member of the Reserve component whp is issued a delayed-effective-date
active-duty order, or is covered by 'such an order, that is for a period of
active duty of more than 30 days in support of a contingency operations, as
defined in 10 USC 101(a) (13) (B), shall be eligible along with the member's
dependents for medical and dental care, on either the date of issuance of
such order or 60 days prior to mobilization, whichever is later.

Additional instructions (cont):
(j) Unit personnel must have copies of marriage license, birth certificate of
children, rental or mortgage agreements and other documents necessary for
pay entitlements to be processed by the mobilization station.
(k) Pursuant to Presidential Executive Order 13223 (14 Sep 2001 ), you are
relieved from your present Reserve Component status and are ordered to
report for a period of active duty not to exceed 25 days for mobilization
processing that includes medical and dental screening and/or care. Proceed
from your present location in sufficient time to report by the date
specified.
(1) If upon reporting for Active Duty you fail to meet deployment medical
standards (Whether because of a temporary or permanent medical condition),
then you willbe released from Active Duty, transfered to your prior reserve
status, and returned to your home address. Subject to a subsequent order
to active duty upon resolution of the disqualifying medical condition.
(m) If upon reporting for Active Duty, you are found to satisfy medical
deployment standards, then you are further ordered to Active Duty for a
period not to Exceed ( 545 ) days. This period of active duty shall
exclude the medicalldental screening period but shall be calculated to
reflect that period.
(n) The soldier will be excluded from the active Army end-strength per
10 USC 115 and will not be placed on the active-duty list
(10 USC 620 and 10 USC 641).
