Register of Wills receives Notice of Inheritance Tax Appraisement, Allowance or
Disallowance of Deductions and Assessment of Tax from the Pennsylvania Department
of Revenue with respect to supplemental Revenue Form 1500 filed on behalf of said
petitioner on this date with the Register of Wills whereupon the Register of Wills will
execute the certificate attached to the petition as Exhibit "A" and thereafter transmit said
petition to the Orphans Court.
