Township Mana er, Louis Fazekas, and files the within claim for charges assessed
with respect to se er and/or water rates against above named owner or owners and
against the herei after described real estate as follows:
STATEMENT OF CLAIM
1. Th s claim is filed by UPPER ALLEN TOWNSHIP, a first class township
organized and ex sting in accordance with the laws of the Commonwealth of Pennsylvania.
2. Th owner or reputed owner of the property, or party or parties
responsible or li le, for which this claim is filed is the Defendant (s).
3. Th authority under which the sewer and/or water rates were charged
and assessed is t e Act of July 18,1935, P.L. 1286. Section 1, 53 P.S. § 2231, as
amended.

6. Thi claim has not been the subject of any previous claim and judgment in
the Court of Co mon Pleas of Cumberland County for sewer/water charges.
7. Th total amount of the charges for which this claim is filed is
Four Hundred, '~'hirty-One and 74/00 dollars ($431.74) plus costs.
8. Th~ time period for which these charges are filed is April 2006 to
March 2007.
RE, Claimant requests that a Municipal Lien be entered in its favor
and against the Defendant in the amount of Four Hundred, Thirty-One and 74/00
dollars ($431.74) plus costs.
