Your Petitioner is counsel for Kirk Kaufinan, natural father of Kobe Kaufman. 2. The said Kobe Kaufinan was involved in an automobile accident on January 4, 2007. 3. The automobile insurance carrier for the driver causing the accident tendered full policy limits to settle the case, a copy of Release Indemnity Agreement is attached hereto, incorporated herein by reference, made a part hereof, and marked as Exhibit A. 4. There was also a lien from DPW of $365.00. x 5. On or about December 17, 2009, the matter was resolved, distribution was made pursuant to the Distribution Sheet, a copy of which is attached hereto, incorporated herein by reference, made a part hereof, and marked as Exhibit B. 6. The matter was discontinued of record on December 23, 2009, as evidenced by the attached Praecipe, marked as Exhibit C. 7. A copy of the Praecipe was forwarded to the insurance carrier. 8. On or about the end of January, 2010, the insurance carrier contacted counsel for the natural father indicating that they now desire a Court Order approving said settlement distribution. WHEREFORE, the Petitioner requests the Court to approve the settlement and distribution that has already occurred in the above-captioned matter pursuant to the Distribution Sheet.
