In reference to the above-captioned Estate and pursuant to the direction of the Court, please find
enclosed a revised Petition for Probate and Grant of Letters together with an Estate Information Sheet,
wherein the date of death is correctly shown as January 21,2007 instead of January 29,2007. These
documents are submitted to obtain reissuance of the Short Certificates to show the correct date of death.
Please send the Short Certificates to our office by using the enclosed envelope.
