North Middleton Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy &
Faller, hereby files its claim for sewer and water services against the real estate hereinafter
described and sets forth its claim as follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed is Duane Wert for 60 Meadowbrook Road, Carlisle, Pennsylvania.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit "A" with an address of 60 Meadowbrook Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16,1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer and water user fees against the above described-property
in the amount of $308.22.

Interest is claimed on the above-mentioned amount from January 31, 2007.

BETWEEN JOSEPH M. BOYER, an unmarried man of West Pennsboro Township,
Cumberland County, Pennsylvania,
GRANTOR
And DUANE E. WERT, JR, an unmarried man of Newville, Cumberland County,
Pennsylvania,
GRANTEE
WITNESSETH, that in consideration of Thirty-five Thousand Five Hundred and no one
hundredths Dollar ($37,500.00), in hand paid, the receipt whereof is hereby
acknowledged, the said Grantor does hereby grant and convey to the said Grantee, his
heirs and assigns,
ALL THAT CERTAIN lot of ground with the improvements thereon erected situate in
North Middletovm Township, Cumberland County, Pennsylvania, bounded and described
as follows:
BEGINNING at a point in the center of the public road leading from the Carlisle-
Newville Road to Meadowbrook Park, said point being the northeast corner of lot of
ground now or formerly of Jahn Shearer, et ux; thence by said lot of ground now or
formerly of John Shearer, et ux., North 88 degrees, 45 minutes, West 160 feet to a point
on line of land now or formerly of Frank S. Cohick, et ux., thence by said land now or
formerly of Frank S. Cohick, et ux., North 1 degree, 15 minutes East 125 feet, more or
less, to a point, said point being the southwest corner of lot of ground now or formerly of
Clair E. Myers, et ux.; thence by said lot of ground now or formerly of Clair E. Myers, et
ux., South 89 degrees 15 minutes East 160.8 feet to a point in the center of said public
road; thence by the center of said public road, South 1 degree, l5 minutes West 125 feet,
more of less, to a point, the place of BEGINNING.
BEING improved with a frame dwelling house known and numbered as 60
Meadowbrook Road, Carlisle, Pennsylvania.

UNDER AND SUBJECT to the same building and other restrictions, but omitting any
such restrictions based on race, color, religion or national origin, referred to in Deed
Book U, Volume 15, Page 384.
BEING TAE SAME PREMISES which Sohn Z. Gibson, widower, by his attorney in
fact, Lee R. Snyder under Power of Attorney dated October 10, 1994 ands rmd ~~the
office of the Recorder of Deeds in and for Cumberland County, Pennsy
Book 498, Page 18I, by deed dated and recorded June 16, 1995 in the Office of the
Recorder of Deeds in and for Cumberland County, Pennsylvatia, in Deed Book 123,
Page 684, granted and conveyed unto Joseph M. Boyer, an unmarried man, Grantor
herein.
AND the said Grantor hereby covenants and agrees that he will warrant specially the
property hereby conveyed.
