PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
Petitioner, Siarhei Alegavich Khodus, respectfully presents:
1. Petitioner, Siarhei Alegavich Khodus, wishes to have his name changed because:
• his real name has lost during translation from Russian to Belarusian then to English
• he wants to adjust his name in observe the U.S. naming customs
2. That petitioner has resided at:
• 3615 Kent Drive, Mechanicsburg, PA 17050, since January, 2006
• 412 Brian Court, Mechanicsburg, PA 17050, since August, 2003
3. That there are no judgments or decrees of like character of record against the petitioner.
WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982, P.L.
1309, No. 295, § 2, 54 Pa. Cons. Stat. Ann. § 702, for the following:
a. To fix a hearing on this Petition;
b. To direct that notice be given on this Petition and hearing; and
c. To enter a decree changing the petitioner's name from Siarhei Alegavich Khodus
to Sergey Oleg Khodus.

ORDER OF COURT
AND NOW, this 24`h day of July, 2007, upon consideration of the attached
Petition For Change of Name, a hearing is scheduled for Wednesday, October 3, 2007, at
2:30 p.m., in Courtroom No. 1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 jHistorical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. §§701 et seq.

IN RE: PETITION FOR CHANGE OF NAME
ORDER OF COURT
AND NOW, this 3rd day of October, 2007, upon
consideration of the Petitioner's Petition for Change of Name, and
following a hearing on the Petition in which the Petitioner
testified and supported the Petition, and it appearing that the
Petitioner has complied with all the requirements of the law with
respect to this Petition, the Court finds that the Petition should
be granted, the Petition for Change of Name is granted, and the
Petitioner, Siarhei Alegavich Khodus, shall hereafter be known by
the name Sergey Oleg Khodus.
