1. Two copies of the completed Rev-1500 Inheritance Tax Return with Schedules A through J
as required.
2. Two copies of the will.
3. A check in the amount of$ 21,538.26 to cover the estimated taxes per item 1 above.
I am requesting an additional six (6) months time to complete the final settlement of the estate at
which time a final adjustment of the estate tax will be submitted.
