LILLIAN C. O'BRIEN, deceased, the amount of ONE THOUSAND ONE HUNDRED
TWENTY-SIX DOLLARS AND EIGHTY-TWO CENTS. ($1,826.82)
2. The nature of the claim is Bon Ton account number 2116041002379858 with a
balance due of $1,459.38 and Kmart account number 2706010100412750 with a balance due of
$367.44.
3. The name and address of the creditor is: ERI Financial Services, P.O. Box 3542,
Baltimore, Maryland 21214.
4. This claim is not contingent and is not secured by any liens or judgments.
5. This claim is not based on anyone instrument. The balance has accrued since the
account was established.
