COMES NOW, Deepak K Sathyanarayanaa, on behalf of Aditya Narayan Deepak, by this
petition and being duly sworn, respectfully represents unto this Honorable Court as follows:
1. That the Petitioner, Deepak K Sathyanarayanan, is the Father of the minor.
2. Aditya Narayan Deepak was born on December 10,2000 in Camp Hill, Pennsylvania.
3. The minor child presently resides at 6113 Wallingford Way, Mechanicsburg, Pennsylvania,
17050. Minor child has lived in Cumberland County, Pennsylvania for 2.7 year(s) immediately
prior to filing this petition.
4. The minor child's present name is Aditya Narayan Deepak. This is the name on his social
security card and all other records.
5. That Aditya Narayan Deepak wishes to change his name to Aditya Deepak Sathyanarayanan.
6. Petitioner desires this change because: Name change is being requested so that child's last
name is kept the same as father's last name.
7. The proposed change of name will not be detrimental to the interests of any other person and
is not against the public interest.
8. The petition is not being filed to defi~aud creditors.
9. That the Child has not been known by another name.
10. The mother of the child is Ramya S Raj who resides at 6113, Walllngford Way,
Mechaniesburg, Pa- 17050.
11. The father of the child is Deepak K Sathyanarayanan who resides at 6113,Wallingford Way,
Pennsylvania, 17050.
12. It is in the best interests of the child to change his name.
WHEREFORE, your Petitioner prays that this Honorable Court pass an Order changing the
