Owen, by himself and respectfully files the following Motor Vehicle Registration Suspension Appeal as follows: 1. Petitioner, Perry L. Owen, is an adult licensed driver with a resident address of 121 Laurel Drive, Enola, Cumberland County, Pennsylvania 17025. 2. The Department of Motor Vehicles has an address of PO Box 68674, Harrisburg, PA, 17106- 8674. Petitioner received notice of proposed 3 month registration suspension for failure of proof of insurance. 3. I wish to appeal this based on the following: a. I did in fact have insurance on my vehicle. I changed insurance companies, from AIG NATIONAL INS CO to PROGRESSIVE. b. I was under the impression that my new insurance company, PROGRESSIVE, would contact the DEPARTMENT OF TRANSPORTATION with my new insurance information. c. This is the first OFFICIAL NOTICE request to provide proof of financial responsibility (insurance) for my vehicle. I plead with the Honorable Court for this Appeal Perry L.
