PETITION FOR NAME CHANGE.
AND NOW, comes Tadeisha T. Burge-Townsend, by and through her Attorney, Jane
Adams, Esquire, and respectfully requests this Honorable Court to change her name to Tadeisha
T. Townsend for the following reasons:
1. Petitioner, Tadeisha T. Burge-Townsend, (hereinafter referred to as "Petitioner")
currently lives at 1321 White Birch Lane, Carlisle, Cumberland County, Pennsylvania, 17013.
2. Petitioner's date of birth is December 7, 1994; she is currently twelve (12) yeazs old.
3. Petitioner currently lives with her mother, Tonda T. Wilkerson, and her step-father,
Don Wilkerson, at the above-address.
4. Prior to living at this address, Petitioner lived at: 6079 Thoroughbred Court, Waldorff,
Maryland, from 2002 through November 2003; and 701 Fleetwood Drive, Papilion, Nebraska
from November 2003 to May 2007.
5. Petitioner's mother and step-father were married on January 2, 2003.
6. Petitioner intends to change her name to Tadeisha T. Townsend.
7. Petitioner's current last name is a hyphenated combination of her natural father's name
and mother's maiden name.
8. Petitioner wants to change her name to Tadeisha T. Townsend because it would be
less cumbersome and she feels more comfortable with the requested last name.
9. Petitioner believes that using her mother's maiden name would avoid confusion
regarding school and medical records.
10. Petitioner's mother joins in this petition and has reviewed the information in this
petition; her verification is attached.

11. Petitioner has limited contact with her father, Todd E. Burge.
12. Petitioner is not asking that notice to natural father be waived under section 54
Pa.C.S.A. §(a)(3)(iii) of the statute.
13. Petitioner is twelve (12) yeazs old and has no outstanding credit debt that could be
avoided through a name change.
14. Petitioner has no outstanding federal or state tax debt that could be avoided through a
name change.
WHEREFORE, Petitioner requests that this Honorable Court Order change her name to:
Tadeisha T. Townsend.
