CIVIL ACTION -COMPLAINT
Plaintiff is a corporation licensed and authorized to conduct business in
the State of Ohio with a place of business located at 4910 Tiedemann Road, Brooklyn,
Ohio 44144.
2. Defendant, Lonnie R. Corte, is an adult individual who at all times
pertinent hereto resided at 513 North Morris Street, Shippensburg, Pennsylvania 17257.
3. On or about May 5, 2004 Defendant entered into a Home Improvement
Installment Contract, "the Contract", with Plaintiff in which Defendant agreed to make
monthly payments. See attached as Exhibit 1, a copy of the Contract.
4. Defendant has failed to make the agreed upon monthly payments and is in
default of the Contract.
5. Despite demand for payment, Defendant refuses to pay the amount owed which
totals $8,443.97 plus contractual interest at the rate of 9.99% from May 20, 2007.

WHEREFORE, Plaintiff demands judgment be entered in its favor and against
Defendant in the amount of $8,443.97 plus contract interest at the rate of 9.99% from
May 20, 2007 plus costs.
