Johnson, Esquire, respectfully represents: 1. Petitioner is Patsy P. Bonanni, who is 59 years of age, born on June 25, 1948, and who resides at 3 James Circle, Shippensburg, Cumberland County, Pennsylvania 17257. 2. Petitioner desires to change his name from Patsy P. Bonanni to Pat Paul Bonanni. 3. The reason for desiring the change of name is Petitioner has always been known by the first name Pat and now wants to legally change it to Pat. 4. During the five years preceding the filing of this Petition, Petitioner has resided at 3 James Circle, Shippensburg, Cumberland County, Pennsylvania 17257. 5. Petitioner will cause to be performed a search of the public records of Cumberland County, Pennsylvania, for judgments or decrees of like character of record against Petitioner. 6. This Petition is filed pursuant to the provisions of the Act of December 16, 1982, 54 Pa. C.S.A. Section 701. 7. A proposed form of Notice for publication is attached to the Petition. WHEREFORE, Petitioner respectfully requests the Court to: a. Schedule a hearing no less than 30 days and no more than 90 days from filing of this Petition; b. Direct that notice be given of the filing of this Petition and hearing by publication in the Cumberland County Legal Journal and one newspaper of general circulation in Cumberland County; and c. Direct the Prothonotary to forward a copy of this Petition along with a set of Petitioner's fingerprints supplied by Petitioner to the Pennsylvania State Police pursuant to 54 Pa. C.S.A. Section 702(b)(1); and d. Direct the Pennsylvania State Police to certify to the Court the action taken by them under 54 Pa. C.S. Section 702(b)(3) within sixty (60) days of the Petition and fingerprints from the Court; and e. After the hearing, change the name of Patsy P. Bonanni to Pat Paul Bonanni. By: z_ Anne S.
