Attached hereto is a statement of the kind and character of the services so rendered and the prices charged therefor, said statement being a true and correct copy of the Owner's account I with the Claimant, which sum of $428.65 was duly assessed against the above Owner and premises beginning July 1, 2006 and for which sum, which includes interest, costs and penalties I and in addition thereto attorney's fees pursuant to Hampden Township Code Ordinances Chapter 18, Part 2, Section 209.6 and Chapter 20, Part 1, Section 106.4, as amended, a lien is claimed against the above premises in accordance with the Municipal Authorities Act of June 19, 2001, LAW OFFICES SNELSAKER & BRENNEMAN, P.C. - .4W .L. 287, 53 P.
