Petitioner is over eighteen (18) years of age. In support hereof Petitioner states as follows: 1. In his original petition, Petitioner sought to change his name to ALVIN Hasancevic. 2. Petitioner subsequently discussed his proposed new name with his wife and others more familiar with popular American culture. Based on these discussions, Petitioner regrets his original choice and now wishes his name to be changed to ERVIN Hasancevic. 3. The substitution of ERVIN for ALVIN is not intended to conceal Petitioner's identity, to confuse or evade third parties, or for any other fraudulent purpose. No credit has been extended to Petitioner under the name Ervin Hasancevic. 4. The substitution of ERVIN for ALVIN does not affect the rights of any party or the timely consideration of Mr. Hasancevic's petition by this court. However, a court order accepting the substitution is a necessary before notice of the proposed name r change can be published. Under the circumstances, amendment of the name change petition nunc pro tunc would be in order.
