PETITION FOR NAME CHANGE
Petitioner, Brooke Elaine Bottorf, a single woman, through her counsel, Humer &
Daniels, petitions this court to legally change her name and in that regard avers the
following:
1. Petitioner is Brooke Elaine Bottorf, of 61 Cherry Lane, Carlisle, Pennsylvania
17015.
2. Petitioner is an adult individual with a date of birth of April 3, 1983 and Social
Security Number 192-66-2453.
3. Petitioner was born in Holy Spirit Hospital at Camp Hill, Pennsylvania, is a
lifelong resident of Cumberland County and has resided at 61 Cherry Lane, Carlisle since
1989.
4. Petitioner has never filed for bankruptcy under 11 U.S.C. §101 et. seq.
5. Petitioner has no judgments or liens of record or any other matter of like
character entered against her or now pending.
6. Petitioner has not at any time made an assignment to the benefit of creditors.
7. There are no claims, demands, liabilities, or obligations of any kind on any
written instrument or otherwise against Petitioner under the name sought to be changed
or any other name except: financial obligation with PHEEA for student loans.
8. Petitioner has no creditors who may be adversely effected or prejudiced by the
proposed change of name.
9. Petitioner seeks a name change to "BROOKLYN GORDON" for the
following reasons:
a. The proposed name is the name that she has used informally since
about April 3, 2001,
b. The proposed first name is what her mother wanted to name her at
birth, and the given name she prefers.
c. She proposes to drop her middle name to simplify things in
anticipation of imminent marriage.
d. The proposed last name is to match the surname of her mother, Tracey
Gordon, and her stepfather, Thomas Gordon, the latter of whom has been "Dad" since
she was two years of age.
e. Her mother maintained full custody of her throughout Petitioner's
minority.
f. She was not adopted because her birth father, Richard E. Bottorf, Jr.,
refused to relinquish parental rights.
WHEREFORE, Petitioner respectfully requests this Court to grant this Petition
and direct Petitioner's name be changed from BROOKE ELAINE BOTTORF to
BROOKLYN GORDON.

Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).
