IN THE MATTER OF
PETITION FOR PAYMENT ASSISTANCE
AND NOW comes Betty M. Petty, by and through her attorney, Lorin Andrew
Snyder, Esquire, and avers the following:
1. Petitioner is represented by court-appointed counsel in the above-
captioned matter.
2. Petitioner requires (1) the release of medical records and (2) an interview
from Dr. Darryl K. Guistwite, D.O., Guistwite Family Practice, 522 South Pitt Street,
Carlisle, PA 17013, (717) 243-1516.
3. The office of Dr. Guistwite requires remuneration quoted in the sum of
$63.14, or thereabouts, for said medical records.
4. The office of Dr. Guistwite requires remuneration quoted in the sum of
$75.00, or thereabouts, for said interview.
4. The Honorable J. Wesley Oler, Jr., has been assigned to this matter.
5. Nathan C. Wolf, Esquire, attorney for Charlotte Brock, Harold S. Irwin, III,
Esquire, attorney for Charles Y. Petty Jr., and Lisa M. Christopher, were contacted and
concur in this motion.
WHEREFORE, Petitioner respectfully requests this Honorable Court to allow
these costs to be paid by the Cumberland County Court System.
