AND VIRGINIA M. T AYLOR Howard and Virginia's needs have required placement in an Assisted Living facility, and Skilled Nursing Facility, resulting in a negative cash flow situation. (See Income and Spending Table attached.) Personal property and contents of the home were sold at auction and proceeds deposited into the Taylors' bank account. After receiving necessary approval from the Court via Petition to Sell Real Estate, the guardian did sell the Taylors' residence and duly deposited the proceeds into their 119147 2 bank account. The guardian rented a storage unit to store additional seasonal clothing, furnishings, and personal effects for Howard and Virginia, due to the limited storage space available to them in the facility. The guardian has paid all bills and obligations incurred by Howard and Virginia, and provided Howard Taylor with cash as needed for excursions outside the facility. The guardian also purchased items to furnish the Taylors' room, as needed, and also undertook the purchasing of new clothing for Virginia and for Howard as he requested. The guardian and alternate guardian also personally transported the Taylors to all medical, dental, hearing, and vision appointments or made arrangements for their transport when necessary. The guardian coordinated and managed the Taylors' mail order prescription plan, resulting in significant savings to them. The guardian also personally shopped for the Taylors, purchasing items such as incontinence products and personal care supplies, hearing aid batteries, and other items as needed. The guardian has also cared for the Taylors' pet dog (medical and grooming needs) and provided Howard and Virginia with many opportunities to visit with their pet which is very important to them. GUARDIAN OF THE PERSON Howard and Virginia Taylor are currently residents of the Masonic Village Healthcare Center, 600 Freemason Drive, Elizabethtown, PA 17022-2199. Howard Taylor On September 13, 2007, Howard was initially placed on the 4th floor Washington Unit to receive skilled nursing services in conjunction with rehabilitative therapy due to 119147 3 a hip injury sustained as the result of a fall. He completed physical and occupational therapies successfully, and currently is placed in the LaFayette East unit where he receives continued skilled nursing care to facilitate ongoing recovery. This unit specializes in Dementia care for higher functioning individuals. Howard may have the opportunity to move to the assisted living section of the facility in the future. Howard continues to display moderate dementia difficulties. Howard has Severe Cardiomyopathy, requiring extensive medication management, including an implanted cardiac defibrillator/pacemaker which will need replacement in the very near future. Howard's other medical conditions are: BPH (benign prostate hypertrophy), osteoarthritis, gout, and severe hearing loss requiring bilateral hearing aids. Howard currently is placed in a semi-private room with another male resident. He receives coordinated medical care thru the resources at Masonic Village, including dental and vision care. He will continue to be seen by his private cardiologist, Jeffrey Fugate, M.D. Howard is currently in the process of evaluations to determine the need for psychological services and counseling, and is expected to begin additional recreational therapies, including aqua therapy in the near future. Howard continues to display impaired memory and reasoning skills and illustrates impaired ability to make judgments to a degree which, in the opinion of the guardian, indicates a continuing need for guardianship. He continues to display total lack of awareness regarding his own cognitive deficits, and also the more severe deficits of his wife Virginia. 119147 4 Virginia Taylor Virginia was placed in the LaFayette West Unit on September 17, 2007, and receives skill nursing care which has become necessary due to her advanced Alzheimer's dementia. She is in a semi-private room with a female roommate. Virginia receives coordinated medical care, including vision and dental care, thru the Healthcare Center at Masonic Village. Virginia continues to experience deteriorating Alzheimer's dementia. She continues to need synthetic thyroid hormones due to ablation of the thyroid, and has recently been diagnosed with Degenerative Joint Disease. As a resident of Masonic Village, Virginia receives coordinated services from doctors, nurses, social workers, recreational therapists, and other medical professionals when indicated. It is the opinion of the guardian that Virginia continues in need of plenary guardianship due to the advance degree of her dementia combined with the fact that her husband is unable to make appropriate decisions for her. During the guardianship, the guardian has visited Howard and Virginia frequently, usually 2-4 times per week from May of 2006 until June of 2007, and two times per month during July and August 2007. During parts of September 2007, visits 119147 5 occurred daily and were extensive. The undersigned guardian understands that false statements made in this inventory are subject to the penalties of 18 Pa. c.s. 94904 relating to unsworn falsification to authorities.
