Comeford residing at 101 November Drive, Apartment #5, Camp Hill, Cumberland County, Pennsylvania 17011. 2. The Defendant is Robert D. Comeford residing at 2653 Walnut Street, Harrisburg, Dauphin County, Pennsylvania 17113. 3. Plaintiff seeks legal custody and primary physical custody of the following child: NAME PRESENT RESIDENCE DOB Liza A. Comeford 101 November Drive 09/22/99 Camp Hill, PA The child was born in wedlock. The child is presently in the physical custody of Claudia M. Comeford who resides at 101 November Drive, Camp Hill, Cumberland County, Pennsylvania. Comeford who resides at 2653 Walnut Street, Harrisburg, Pennsylvania 17113. He is single. 4. The relationship of Plaintiff to the child is that of mother. The relationship of Defendant to the child is that of father. The Defendant is currently residing with: Unknown. 2 6. Neither party has participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another court. Plaintiff does not know of a person not a party to the proceedings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. 7. The best interest and permanent welfare of the child will be served by granting the relief requested because: A. Mother has been the child's primary caretaker B. Father has not been involved in child's life. C. Mother will place her child's best interest before her own. D. Father has been alleged to have had improper physical contact with his daughter. 8. Each parent whose parental rights to the child has not been terminated and the person who has physical custody of the child have been named as parties to this action. WHEREFORE, Mother requests this Honorable Court to grant her legal custody and primary physical custody of Liza, subject to Father's right to periods of supervised visitation. Verney, Esq. , the conciliator, at 4th Floor, Cumberland County Courthouse, Carlisle on Tuesday, December 04, 2007 at 9:30 AM for a Pre-Hearing Custody Conference. At such conference, an effort will be made to resolve the issues in dispute; or if this cannot be accomplished, to define and narrow the issues to be heard by the court, and to enter into a temporary order. All children age five or older may also be present at the conference. Failure to appear at the conference may provide grounds for entry of a temporary or pennanent order.
