TO THE HONORABLE JUDGES OF SAID COURT: Petitioner, Ashley Jason Owen, by and through his attorney, Kristina A. Bange Esquire, hereby petitions this Honorable Court to enter a decree changing his name from ASHLEY JASON OWEN to ASHTON JAY OWEN, and in support thereof respectfully represents the following: 1. Petitioner, Ashley Jason Owen, is an adult male individual, who was born on May 19, 1979, and is over the age of 21 years. 2. It is Petitioner's intention to change his name. 3. During the five years proceeding the filing of this Petition through and including the date hereof, Petitioner has resided continuously at 136 Bridge Road, Newville, Cumberland County, Pennsylvania 17241. 4. There are no judgments or decrees of record or other similar matters against the Petitioner in this or any other jurisdiction. 5. Petitioner requests that his name be changed from Ashley Jason Owen to ASHTON JAY OWEN, for the following reasons: a. Petitioner dislikes the name "Ashley" because most people believe it is a girl's name, and he is often mistakenly referred to as "Miss Owen" or "Ms. Owen." 7 b. Petitioner's choice for his new name would leave him with the same initials (A.J.O.) as his present name, therefore minimizing the possible confusion surrounding a name change. c. Petitioner's choice for his new middle name is his father's first name. WHEREFORE, Petitioner, respectfully requests this Honorable Court to: a. Set a time and place for a hearing on this Petition; b. By Order, direct that notice be given of the filing of the Petition and of the date set for the hearing on the Petition; c. Change Petitioner's legal name, following a hearing, from Ashley Jason Owen to ASHTON JAY OWEN.
