PETITION FOR CHANGE OF NAME
1. Petitioner is Stephanie Vogel, a dependent child in the care and custody of
Cumberland County Children & Youth Services.
2. Petitioner, who was born September 16, 1985, has been in placement for
mom than ten years and currently resides in a foster home at 354 Roxbury Road,
Newville, PA, 17241.
3. Petitioner has resided at 354 Roxbury Road, Newville, PA, for the past
two and one-half years with Carol and Warren Jenkins. Prior to that time, petitioner
resided at Meadows Psychiatric Center at Centre Hall, PA, 16828, for a year from
November 17, 1999 to October 2000. Prior to November 17, 1999, petitioner resided at
the Childrens Aid Society in Chambersburg. Prior to residing at the Childrens Aid
Society, petitioner lived in a Mentor Foster Home in Mercersburg, PA.
4. Petitioner's mother is Debra Spangler, who resides in New York and
whose mailing address is c/o General MacArthur, P.O. Box 794, New York, NY, 12754.
Petitioner has not seen her mother since 1994 when she attended the funeral of her sister
in New York.

Petitioner's father, Richard Vogel, is deceased.
Petitioner lives in the foster home of Carol and Warren Jenkins and
intends to continue residing with them and wishes to change her name from Vogel to that
of Jenkins.
7. Warren and Carol Jenkins, the foster parents, consent to the change of
petitioner's name to Stephanie Jenkins.
8. Cumberland County Children & Youth Services also agree to the change
of petitioner's name to Stephanie Jenkins.
WHEREFORE, petitioner requests that the Court enter a decree changing her
name from Stephanie Vogel to Stephanie Jenkins.

OF PROPOSED NAME CHANGE

petitioner, it is ORDERED that a hearing on the Petition for Change of Name is fixed for
