1. Plaintiff, CHASE BANK USA, N.A. is a corporation with offices at
3700 WISEMAN BLVD. SAN ANTONIO , TX 78251 .
2. Defendant , is adult individual(s) residing at the address listed
below:

3. Defendant applied for and received a credit card issued by
Plaintiff bearing the account number 4366163026264404 .
4. Defendant made use of said credit card and has a current balance
due of $23932.87 .
5. Defendant is in default of the terms of the Cardholder Agreement
having not made monthly payments to Plaintiff thereby rendering the
entire balance immediately due and payable.
6. Plaintiff avers that the Cardholder Agreement between the parties
provides that Defendant will pay Plaintiff's attorneys' fees.
7. Plaintiff avers that such attorneys' fees will amount to $1500.00
8. Although repeatedly requested to do so by Plaintiff, Defendant has
willfully failed and/or refused to pay the principal balance, and
accrued interest or any part thereof to Plaintiff.
Wherefore, the Plaintiff prays for judgment against Defendant ,
SCOTT E BASH , INDIVIDUALLY , the amount of $23932.87 with continuing
interest thereon at the statutory rate of 6.000% per annum from the
date of judgment, plus attorneys fees of $1500.00 and costs.

To gel the daily balance for each day of the current billing cycle, we take the
beginning balance for each feature, add any now transactions or other debits

payments or credits, and make other adjustments. Transactions are added
as of the transaction date, the beginning of the billing ryde in which they an
posted to your account, or a later date of our choice (except that check
transactions are added as of the date deposited by tin payee or a later date
of our choice). Fees are added either on the date of a related transaction, the
date they are posted to your account, or the last day of the billing cycle. This
gives us that day's daily balance. A credit balance is treated as a balance of

that day. We then add these periodic finance charges to your day balance

Grate Period (at lass 21 Age):
We accrue periodic finance charges on a transaction, in, or finance charge
from the date it Is added to your day balance until payment in full is
received an your account. However, we do not charge periodic finance
charges an now purchases billed during a billing cycle if we receive both
payment of your New Balance on your current statement by the date and
time your payment is due and also payment of your New Balance on your
previous statement by the date and time your payment was due. There Is no
grace period for balance transfers, cash advances, dark transactions, or
overdraft advances.
