AND NOW comes Petitioners, MUHAMMAD CHAUDRY ASLAM, KULSOOM
AKHTER, MUHAMMAD HASSAN ASLAM, MUHAMMAD HASEEB ASLAM and MUHAMMAD
HAMMAD ASLAM, by and through their attorney, Thomas D. Gould, and
requests this Honorable Court, pursuant to 54 P.S. Section 701, to
issue a Decree changing the name of Petitioners to CHAUDHRY M.
ASLAM, KULSOOM A. ASLAM, HASSAN M. ASLAM, HASEEB M. ASLAM and
HAMMAD M. ASLAM based on the following:
1. Petitioner MUHAMMAD CHAUDRY ASLAM is an adult citizen who
resides at 4800-A Charles Road, Mechanicsburg, Cumberland County,
Pennsylvania.
2. Petitioner is 60 years of age having been born on
February 6, 1947.

3. Petitioner KULSOOM AKHTER is an adult citizen who resides
at 4800-A Charles Road, Mechanicsburg, Cumberland County,
Pennsylvania.
4. Petitioner is 46 years of age having been born on April
6, 1961.
5. Petitioners MUHAMMAD HASSAN ASLAM (DOB 06/22/91),
MUHAMMAD HASEEB ASLAM (DOB 06/19/93) and MUHAMMAD HAMMAD ASLAM
(DOB 11/14/94) are the minor children of petitioners Mohamed
Chaudry Aslam and Kulsoom Akhter.
6. During the last five (5) years the Petitioners have
resided at the following locations:
4800-A Charles Road, Mechanicsburg, PA 10/04 - present
907 Middle Street, Gap, PA 03/03 - 10/04
909 Roberts Road, Bensalem, PA 1999 - 03/03
7. Petitioners desires to change their names to avoid
confusion and more accurately represent their family heritage.
8. Petitioners have no criminal charges, liens, judgments or
decrees of record or any other matter pending of like character
against them in this or any other jurisdiction.
9. The Petitioners Mohamed Chaudry Aslam and Kulsoom Akhter
are married and concur in this petition.
WHEREFORE, Petitioners respectfully request this Honorable
Court to set this matter for hearing and enter an Order directing
Petitioners to give Notice of the filing of this action and the
date and time set for Hearing, which shall not be less than one
month or more than three months after the filing of this Petition.

After hearing it is respectfully requested that this Honorable
Court issue a Decree changing the name of Petitioner, MUHAMMAD
CHAUDRY ASLAM to CHAUDHRY M. ASLAM, petitioner KULSOOM AKHTER to
KULSOOM A. ASLAM, petitioner MUHAMMAD HASSAN ASLAM to HASSAN M.
ASLAM, petitioner MUHAMMAD HASEEB ASLAM to HASEEB M. ASLAM and
petitioner MUHAMMAD HAMMAD ASLAM to HAMMAD M. ASLAM.
