PETITION FOR CHANGE OF NAME
NOW comes the petitioner, THOMAS EDWARD McMANUS, by his attorney, Harold S. Irwin, III,
Esquire, and presents this petition for change of name, representing as follows:
1. Petitioner is THOMAS EDWARD McMANUS, also known as THOMAS FOX McMANUS,
an adult individual residing at 75 Long View, Carlisle, Cumberland County, Pennsylvania 17013
2. Petitioner married Lisa Jeanne Fox on May 20, 1995, and after his marriage assumed
the name Thomas Fox McManus.
3. Subsequently, petitioner changed his name officially for all significant matters, including
creditors, social security, his employment identification and security clearance, real estate
ownership, for professional publications and for other matters.
4. It is now necessary for petitioner to change his name formally as he has been unable to
change his name on his driver's license and passport but desires to so that all of his official
documentation is consistent with the name that he has assumed.
5. There are no judgments or decrees of like character of record against the petitioner,
except for his mortgage which is already in the name of Thomas Fox McManus.
WHEREFORE, petitioner requests your Honorable Court, pursuant to Act of December 16,
1982, P.L. 1309, No. 295, Section 2, 54 Pa. Cons. Stat. Section 701, et seq., to do the
following:
A. Fix a hearing on this petition; and
B. Enter a decree changing the name of petitioner from Thomas Edward McManus
to Thomas Fox McManus.
