Kindly file the attached Notice of Resumption of Surname:
Helen Haulman by her intent to resume use of the surname Witmer, pursuant to Pa. Cons.
Stat. §704, and states as follows:
1. Helen Haulman is an adult individual residing at 208 North Mountain Road,
Carlisle, Pennsylvania, County of Cumberland, Commonwealth of Pennsylvania.
2. Helen Haulman was married to Donald W. Witmer and divorced therefrom on
October 25, 1978, which divorce is docketed at No. 1978-1403 Civil Term.
3. Helen Haulman was then married to John W. Haulman and divorced therefrom on
June 11, 1980, which divorced is docketed at No. 1980-501 Civil Term.
4. At all times prior to her marriage to John W. Haulman, she used the surname
Witmer.
5. From this date forward, she will use the surname Witmer.
