JoANN BENSON, being duly sworn according to law, deposes and says
that she is the Defendant in the above suit in which a Final Decree of Divorce
from the bonds of matrimony was entered on the 11th day of August, 1981, that
JoANN BENSON elects to retake and hereafter use her maiden name (or prior
name) JoANN C. RENSHAW and therefore, gives this written notice avowing
said intention in accordance with the provisions of the Act of May 25, 1939,
P.L. 192, as amended.
