NOTICE IS HEREBY GIVEN that Cathy Lou Ortiz, a/k/a Kathy Ortiz, the Defendant in the
above matter, having been granted a Final Decree in Divorce on June 3, 1981, hereby intends to
resume and hereafter use the previous name of CATHY LOU TALLEY, and gives this written notice
avowing her intention in accordance with the provisions of the Act of December 6, 1982, P.L. 1309,
54 Pa. C.S.A. §704, effective March 6, 1983.
