of the Petition for Appointment of Guardian for the Estate of a Minor and for Approval of a
Trust for Said Minor, it is hereby
ORDERED AND DECREED that Michael Langan is appointed as the Guardian of the
Estate of the Minor Child, Jesse A. Zeigler; and moreover
ORDERED AND DECREED that Michael Langan is authorized, as Trustee of the minor
child, Jesse A. Zeigler, to open a trust account in the form of a federally insured certificate of

deposit at Members First Federal Credit Union, in the name of Jesse A. Zeigler, Michael Langan,

as trustee, in the gross amount equal to that now contained in that certain CIGNA Insurance
Group account now held for the benefit of the minor child, Jesse A. Zeigler; and moreover
ORDERED AND DECREED that CIGNA Insurance Group shall turn over to Michael
Langan all funds being held by it for the benefit of said Jesse A. Zeigler, to be deposited into the
trust account contemplated herein; and moreover
ORDERED AND DECREED that said trust account shall be for the benefit ofthe minor
child, Jesse A. Zeigler, and that such account shall contain the following restrictive language
regarding withdrawals: "Payments may be made as necessary from the principal and/or interest of
the trust solely for the health, education and welfare of the minor upon Court Order. The

remainder of the trust proceeds shall be paid to the minor when the minor reaches 18 years of
age."
IT IS FURTHER ORDERED AND DECREED that the proceeds are to be placed in a
