Your Petitioner is Lilian Ngo; she is an adult individual. She resides at 205 Fineview Road, Camp Hill, Pennsylvania 17011; she is desirous of being appointed guardian of the person and estate of Mindy Ngo, the subject of this petition. 2. The subject of this petition is Mindy Ngo, a minor, who was born on December 13, 1988, and hence she is presently twelve years old. A copy of her birth certificate is attached hereto, marked Exhibi t A and incorporated herein by reference. Mindy Ngo has lived with Petitioner since 1997 when Mindy moved to the United States. 3. The natural mother of Mindy Ngo is Chi Do; she is an adult individual. Huong Ngo is the natural father of Mindy Ngo; he is an adult individual. His residence address is 205 Fineview Road, Camp Hill, Pennsylvania 17011. Chi Do also resides at 205 Fineview Road, Camp Hill, Pennsylvania 17011. Huong Ngo, the natural father, consents to the proposed guardianship. Chi Do, the natural mother, consents to the proposed guardianship. A consent as signed by Chi Do is attached hereto, marked Exhibit Band incorporated herein by reference. A consent as signed by Huong Ngo 1 is attached hereto, marked Exhibit C and incorporated herein by reference. 4. Your Petitioner is not the parent of any children, either by birth or by adoption. S. There has been no prior action for the appointment of a guardian involving the said Mindy Ngo. The proposed guardian has no adverse interest to the minor and the minor has no interest in any estate or assets. 6. The religious affiliation of the proposed guardian Lilian Ngo and the minor child, is the same in that they are sisters. 7. The proposed guardian is an information system engineer for Vanguard at Malvern, Pennsylvania. The proposed guardian speaks, reads and writes the English language. The proposed guardian is a citizen of the United States of America; the proposed guardian is not a fiduciary, or officer or employee of a corporate fiduciary, or of an estate in which the minor has interest. Nor is the proposed guardian a surety, or officer or employee of a corporate surety, of such a fiduciary. 8. It is your Petitioner's desire that guardianship as described above be decreed because the minor's natural parents are in failing health, and it is actually the Petitioner who raises the minor from day to day.
