PETITION FOR THE APPOINTMENT OF A GUARDIAN
FOR A MINOR PLAINTIFF. ANTHONY JUSTIN APGAR
AND NOW, COMES the Petitioner, ANTHONY mSTIN APGAR, and respectfully
represents the following:
1. The subject minor child is named ANTHONY mSTIN APGAR.
2. ANTHONY mSTIN APGAR is a minor ofthe age of fifteen (15).
3. ANTHONY mSTIN APGAR resides at 70 West South St., Apt. 2, Carlisle,
Pennsylvania.
4. TAMMY LYNN APGAR, ANTHONY mSTIN APGAR'S natural mother resides at
70 West South ST., Apt 2, Carlisle, Pennsylvania.
5. ANTHONY mSTIN APGAR' natural father is unknown.
6. The Petitioner, ANTHONY mSTIN APGAR, is the minor child.
7. The Petitioner, ANTHONY mSTIN APGAR, resides at 70 West South St., Apt. 2,
Carlisle, Pennsylvania.
8. TAMMY LYNN APGAR, ANTHONY mSTIN APGAR'S natural mother, consents
to this Petition.
9. TAMMY LYNN APGAR, ANTHONY JUSTIN APGAR'S natural mother, executed
a consent to the appointment of a guardian. A true and correct copy of the same is
attached hereto.
10. The appointment of a Guardian is required as TAMMY LYNN APGAR ANTHONY
JUSTIN APGAR'S, natural parent ofthe minor child, is allowing her son,
ANTHONY JUSTIN APGAR to relocate to North Carolina, and attend school there
in Lexington, North Carolina, as a result of disciplinary problems in Pennsylvania.
11. The proposed Guardian is, BEVERLY ANN WILLIS, age 56.
12. The proposed Guardian, BEVERLY ANN WILLIS, resides at 231 Creekside Lane,
Lexington, North Carolina.
13. The proposed Guardian, BEVERLY ANN WILLIS, is the maternal grandmother of
ANTHONY JUSTIN APGAR.
14. The proposed Guardian, BEVERLY ANN WILLIS, has no interests which are
adverse to ANTHONY JUSTIN APGAR.
15. The proposed Guardian, BEVERLY ANN WILLIS, is Methodist.
16. ANTHONY JUSTIN APGAR has been baptized and is considered Methodist.
17. ANTHONY JUSTIN APGAR does not have an estate that requires an itemization.
18. Petitioner is not aware of any entitlement ANTHONY JUSTIN APGAR may have as
party to any action.
19. ANTHONY JUSTIN APGAR is not the child of a veteran, therefore notice of this
petition to the United States Veterans' Administration or its successors is not
required.
20. The proposed Guardian, BEVERLY ANN WILLIS, has consented to her appointment
of Guardian as evidenced by the attached consent.
WHEREFORE, Petitioner respectfully requests that this Honorable Court grant
BEVERL Y ANN WILLIS guardianship of the person of ANTHONY mSTIN APGAR.

guardianship. I have reviewed the petition for guardianship and consent to the appointment of
BEVERLY ANN WILLIS as the guardian for our child. I fully understand the ramifications of
my decision and I fully believe that the appointment of BEVERLY ANN WILLIS as the
guardian for my child is in my child's best interest.
