I apologize for the delay in returning this form.
I am no longer at the Letchworth Road address and my mail is being forwarded to Sioux Falls, SO.
However, since the letter was addressed to me as Jacqueline Gloss (which was my name prior to 1991),
the Post Office forwarded it to my son (whose name is Gloss) in North Carolina. Because he was on the
road, it took a while to catch up to him. When it did, he called me and I asked him to open it and then
