upon consideration of the petition of Barbara G. Lawver, it is hereby ordered and decreed that
Barbara G. Lawver is appointed successor custodian for Ian M. Smith, a minor, of the securities
in the Salomon Smith Barney Account No. 724-21981-12 790 now held in the name of Larry G.
Lawver as custodian under the Pennsylvania Uniform Transfers to Minors Act for the benefit of
Ian M. Smith, a minor.

PETITION FOR APPOINTMENT OF SUCCESSOR CUSTODIAN UNDER THE
PENNSYLVANIA UNIFORM TRANSFERS TO MINORS ACT IN ACCORDANCE
WITH 20 PA. CONS. STAT. ~ 5318
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petitioner, Barbara G. Lawver, represents that:
1. Petitioner resides at 2715 Laurel Street, Cumberland County, Pennsylvania, and is the
grandmother of Ian M. Smith, a minor, born January 17, 1995, now six years of age.
2. During his lifetime, Larry G. Lawver, petitioner's spouse and the grandfather of the minor,
made a transfer to the minor of certain securities and designated himself as "custodian" to Ian M.
Smith under the Pennsylvania Uniform Transfers to Minors Act."
3. Said transferor of the securities, Larry G. Lawver, died on February 18,2001, leaving
petitioner his entire estate, and without designating a successor custodian.
4. The minor resides with his father and mother at 2718 High Street, Grantham, Cumberland
County, Pennsylvania.
5. No guardian has been appointed for the estate of the minor.
6. The securities are presently held in a Salomon Smith Barney account numbered 724-21981-12
790 and have an approximate current fair market value of less than $3,000.00.

7. Petitioner desires to be appointed successor custodian to Larry G. Lawver of the
aforementioned securities.
WHEREFORE, petitioner,Barbara G. Lawver, requests that the Court appoint her as successor
custodian for Ian M. Smith, a minor, under the provisions of the Pennsylvania Uniform Transfers
to Minors Act of the aforesaid securities presently held in the name of Larry G. Lawver as
custodian.
