DECREED, this 111~ day of r:~~'rU().f/ , 2008, on consider,atlon of1he annexed petition and after hearing held following due notice, it is hereby ORDERED and [\.1 \.:~ DECREED that, based on a finding that Tracy A. McHale suffers from a closed head injury, and as a result, suffers from posttraumatic dementia, she is mentally impaired from participating in rational decisions relating to her Metropolitan Life Insurance Company Annuity structured settlement and she is adjudicated partially incapacitated. Michael and Karin McHale, parents of Tracy A. McHale, are appointed limited guardians of the estate of Tracy A. McHale for the purpose of making any and all decisions relating to her Metropolitan Life Insurance Company Annuity structured settlement. Within twelve (12) months of the date of this Decree and at least annually thereafter, the guardians shall file with this Court a report that includes all infom1ation as required pursuant to 2(J Pa. Cons. Stat. Ann. ~5521 (c). Tracy A. McHale has twenty (20) days from the date of this Decree to file exceptions. Failure to file exceptions within that time will result in this Decree becoming final. Tracy A. YOU HAVE THE RIGHT TO FILE EXCEPTIONS TO THE COURT'S DECISION WITHIN TWENTY (20) DAYS OF THE DATE OF THE COURT'S ORDER. IF YOU FAIL TO FILE EXCEPTIONS, THE ORDER WILL BECOME FINAL.
