HomeMy WebLinkAbout04-0179IN THE MATTER OF PETITION FOR CHANGE OF NAME OF: JAMES VINCENT WITT IN THE COURT OF COMMON PLEAS OF CUMBERLAND COUNTY, PENNSYLVANIA NO. 04- I'?~ CIVIL ACTION - LAW CHANGE OF NAME PETITION FOR CHANGE OF NAME AND NOW, comes the Petitioners, Linda Ann Witt and Patrick Raymond Lilly, parents of James Vincent Witt, by and through their attorneys, MARTSON DEARDORFF WILLIAMS & OTTO, and requests Your Honorable Court to order a change of name for their minor son, James Vincent Witt, pursuant to 54 Pa. C.S. §701 et. seq. and in support thereof aver as follows: 1. Petitioners are Linda Ann Witt, and Patrick Raymond Lilly, parents of James Vincent Witt, and bring this action on behalf of their minor son. 2. Petitioner Linda Ann Witt is the mother and primary custodian of the minor, and resides at 83A Partridge Circle, Carlisle, Cumberland County, Pennsylvania. 3. Petitioner Patrick Raymond Lilly is the father of the minor and resides at 513 Guy Road, Philipsburg, New Jersey. 4. The Petitioners never married. 5. The minor, James Vincent Witt was born on March 27, 1990, in New Jersey. His birth certificate is attached hereto as Exhibit "A." 6. The minor and Petitioner Linda Ann Witt moved to Cumberland County, Pennsylvania in 1997, and have resided here ever since. 7. Your Petitioners desire to change the minor's name from James Vincent Witt to James Vincent Patrick Lilly because of the minor's strong desire to take his father's last name. 8. There are no judgments or decrees of like character of record against the minor, nor any criminal convictions or pending criminal charges. WHEREFORE, Petitioners request Your Honorable Court to enter a decree changing the name of James Vincent Witt to James Vincent Patrick Lilly.
