service of a copy of the Statement of Rights Upon Appointment of a Guardian, was made on

Golden Living Center. 46 Erford Road, Camp Hill. P A. 17011. I read the Statement of Rights to
the alleged incapacitated person to the maximum extent possible, in language and terms she was
