PETITION FOR CHANGE OF NAME
AND NOW, comes Andrew Charles Letterman to in support of his Petition for a name
change pursuant to 54 Pa. Const. Stat. Ann. Section 701 et seq.
Avers as follows:
1. Andrew Charles Letterman was bom July 5, 1985. His Social Security Number is
190-66-3249.
2. Petitioner's current address is 217 Wood Street, Camp Hill, PA 17011.
3. Petitioner wishes his name to be changed to Andrew Charles Mshar.
4. Mshar is the Sir name of Petitioner's mother and step-Pather with whom he lives.
5. Petitioner is currently a student in Pennsylvania State University at State College,
Pennsylvania.
6. Petitioner has no Judgements or Mortgages against him, nor does he have any criminal
record.
7. Petitioner believes that it would be in his best interest to have his name changed to
Andrew Charles Mshar.
WHEREFORE, Petitioner respectfully requests the Court order that his name be changed
to Andrew Charles Mshar.

In accordance with the Name Change Act of 1998, the fingerprint cards of the above named individual, who
has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:
This person is not subject to 18 PA C.S. Chapter 91, and the fingerprints have been destroyed.
( ) This person is subject to 18 PA C.S. Chapter 91, and the name change has been noted on the person's
criminal history record information.
( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SItALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN
A NAME CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The
Pennsylvania State Police, upon receipt of this notice, shall include the change of name information in the
Central Repository as provided for in 18 PA C.S., Chapter 91 (relating to criminal history record
information).

publication of notice as required by law, together with proof that there are no judgments or
decrees of record or any other matter of like character against petitioner, and it appearing that
there is no lawful objection to the request of the petitioner, it is hereby ORDERED that the name
of petitioner Andrew Charles Letterman is changed to Andrew Charles Mshar.
