Petitioner, Jessiea Made Tomaszewski, is an adult individual residing at 23 West High Street, Apartment #3, Carlisle, Cumberland County, Pennsylvania. 2. Petitioner has resided at 23 West High Street, Carlisle, Cumberland Coumy, Pennsylvania, for approximately one (1) year. 3. Petitioner's date of birth is June 4, 1983. 4. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6) months preceding the filing of this Petition. 5. Petitioner desires to change her last name from Tomaszewski to Thomas. 6. Petitioner has no relationship with her natm-al parents and, in fact, was a victim of abuse by one of her natural parents and, as such, Petitioner wishes to cut ties with her natural parents. 7. Petitioner desires to attend medical school and she believes that by shortening her name, it will be easier for her patients and others to pronounce and spell. 8. Petitioner has been using the surname of Thomas on her artwork since she was in eighth grade, SAIDIS SHUFF, FLOWER & LINDSAY 26 W. High Streel 9. Petitioner now desires to legally take the surname of Thomas, thereby legally changing her name to Jessica Made Thomas. 10. Petitioner does not wish to change her name for any illegal purpose or to avoid any obligation(s). Petitioner has been fingerprinted by the Pennsylvania State Police and her fingerprint card is attached hereto as Exhibit "A" and by reference incorporated herein. Honorable Court issue an Order legally changing her name Tomaszewski to Jessica Made Thomas. 11. Petitioner has never been convicted of a felony. 12. Petitioner's Social Security number is 040-76-0827.
