The relationship ofthe petitioner to the decedent is daughter. ", ) 3. The decedent, BETTY K. MCQUAID, widowed, died on November 20,2007. (~ 4. Decedent was domiciled at death in Cumberland County, Pennsylvania with a last family or principal residence at 7508 Wertzville Road, Carlisle, Pennsylvania 17013. 5. Decedent's Social Security Number was 203-20-9281. 6. The death certificate is attached hereto. 7. The decedent, BETTY K. MCQUAID, died owning property of a gross value not exceeding Twenty-five Thousand Dollars ($25,000) and which is comprised of forty-five (45) shares of MetLife with a date of death value of $62.44 per share for a total value of $2,809.80. 8. The Pennsylvania Transfer Inheritance Tax Return reporting said forty-five (45) shares of MetLife was filed with the Register of Wills on February 21,2008, and petitioner has paid the inheritance tax shown to be due. - 1 - 9. Elizabeth M. McQuaid, daughter, is entitled to, and claims, the family exemption under Section 3121 of the PEF Code by virtue of being a member of the same household as the decedent. Therefore, Elizabeth M. McQuaid is entitled to the forty-five (45) shares of MetLife or the proceeds thereof as claimant of the family exemption. 10. With the exception of Paragraph 9 above, there are no other unpaid claimants of whom petitioner has notice or knowledge. 11. All heirs/parties in interest of the decedent are listed below. McQuaid who died on November 20,2007, be made to the decedent's daughter, Elizabeth M. McQuaid. AND the said Elizabeth M.
