PETITION FOR NAME CHANGE
reasons:
Petitioner, Marjory Elizabeth Boyd, a married woman, through her counsel, Reager & Adler, PC,
petitions this court to legally change her name and in that regard avers the following:
1. Petitioner is Marjory Elizabeth Boyd, 899 Oak Oval, Mechanicsburg, Cumberland County,
Pennsylvania 17055.
2. Petitioner is an adult individual with a date of birth of January 4, 1925 and Social Security
Number 514-16-2125.
3. Petitioner has resided at 899 Oak Oval, Mechanicsburg, Cumberland County, Pennsylvania
since September 2003. Prior to that date, Petitioner resided at 1145 Country Club Road, Camp Hill,
Cumberland County, Pennsylvania 17011 and 430 NW 30th Street, E1 Dorado, Kansas 67042.
4. Petitioner has never filed for bankruptcy under 11 U.S.C. § 101 et. seq.
5. Petitioner has no judgments or liens of record or any other matter of like character entered
against her or now pending.
6. Petitioner has not at any time made an assignment to the benefit of creditors.
7. There are no claims, demands, liabilities, or obligations of any kind on any written instrument
or otherwise against Petition under the name sought to be changed or any other name except: financial
obligation with Messiah Village for residence.
8. Petitioner has no creditors who may be adversely effected or prejudiced by the proposed
change of name.
Petitioner seeks a name change to "MARJORIE ROSANN BOYD" for the following
The proposed name is the name that she has used for most of her life.
She believes that her birth name is Marjorie Rosarm Vestring (maiden name),
although the existing birth records indicate otherwise.
WHEREFORE, Petitioner respectfully requests this Court to grant this Petition and direct
Petitioner's name be changed fi.om MARJORY ELIZABETH BOYD to MARJORIE ROSANN BOYD.

Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
