MISCELLANEOUS PETITION TO ADD A MIDDLE NAME
1. I, Fotini Skouvakis, residing at 48 West Pomfret Street, Carlisle, Pennsylvania,
petition this court pursuant to Pa.R.C.P. No. 206.1 to add a middle name to my
already existing legal name.
2. My date of birth is June 20, 1980.
3. I currently do not have a middle name and I am petitioning this court to add the
middle name ANTONIA.
4. The purpose of this request is that I was recently married on December 27, 2003
and would like to preserve a part of my maiden name as my middle name.
5. I am not adding this middle name to escape any criminal or civil allegations or
charges.
6. I petition this court to enter an order changing my current name, Fotini Skouvakis,
to FOTINI ANTONIA SKOUVAKIS.
I, Fotini Skouvakis, this 23rd day of January, 2004, verify that the facts set forth are
true and correct to the best of my knowledge, information and belief. I so state subject
to the penalties of 18 Pa.C.S. §4904.
