NAME CHANGE
PETITION FOR NAME CHANGE
AND NOW, comes Lloyd E. Shoemaker, III, parent and natural guardian of Camden
C. Verdekal, and petitions this Honorable Court as follows:
1. Petitioner is Lloyd E. Shoemaker, III who resides at 19 Countryview Estates,
Newville, Cumberland County, Pennsylvania 17241. He is the parent and natural guardian of
a son, Camden C. Verdekal, born March 5, 2004.
2. The mother of the child is Maria E. Verdekal whose mailing address is P.O.
Box 191, Carlisle, Cumberland County, Pennsylvania. Camden C. Verdekal was born out of
wedlock. A copy of his birth certificate is attached hereto as Exhibit "A".
3. Petitioner has custody of Camden C. Verdekal and has had custody since the
child was a year of age.
4. Petitioner seeks to change the name of the child to Camden C. Shoemaker in
anticipation of the beginning of school. The child calls himself Camden Shoemaker.
5. Maria Verdekal does not object to the change of name for the child. A copy of
her Consent is attached hereto as Exhibit "B".
6. Petitioner prays this Honorable Court to hold a hearing and, after the hearing,

CONSENT TO NAME CHANGE
I, Maria E. Verdekal, consent to the change of name of my child, Camden C. Verdekal,
to Camden C. Shoemaker. This change of name will give him the last name of his father.

consideration of the within Petition, a hearing is set for the -? day of

Notice of filing of the Petition and the hearing date shall be made by the Petitioner in
a newspaper of general circulation in Cumberland County, Pennsylvania, and by publication
in the Cumberland County Law Journal. Further, notice of the Petition and hearing shall be
provided to the nonpetitioning parent, Maria E. Verdekal.
At the hearing, Petitioner shall present to the Court proof of publication of the notices
required herein and an official search of the counties in which the child has resided within the
last five years for any judgments or decrees against the child.
