Letters Testamentary of which the following is a statement: \,CI
I. Decedent is Anna M. Cullings, late of Silver Spring Township, Cumberland
County, Pennsylvania.
2. Decedent died on July 31,2007. See copy of death certificate on file.
3. On August 7,2007, Harry M. Cullings, James D. Cullings, and Karen S. Cullings
filed a Petition for Probate and Grant of Letters in the Estate of Anna M. Cullings, Deceased.
4. On August 20,2007, Letters Testamentary were issued to Harry M. Cullings,
James D. Cullings, and Karen S. Cullings, as Co-Executors of the Estate of Anna M. Culling:;,
Deceased.
5. The Petition for Probate and Grant of Letters was prepared by the undersigned.
6. The Petition for Probate and Grant of Letters erroneously set forth the date of
death of Anna M. Cullings as July 30,2007.

7. As a result of the error in the Petition for Probate and Grant of Letters, the
Letters Testamentary issued to the Co-Executors erroneously set for the date of death as July
30,2007.
8. The correct date of death of Anna M. Cullings is July 31, 2007, and your
Petitioner files this Petition seeking that the Letters Testamentary be corrected and amended to
reflect that date.
WHEREFORE, your Petitioner prays Your Honorable Court for an Order correcting
and amending the Letters Testamentary in the Estate of Anna M. Cullings, Deceased, to
properly show the date of death as July 31,2008.
