'~/,.~ CIVIL DIVISION - LAW COMPLAINT Plaintiff is a corporation duly licensed and authorized to conduct the business of insurance in the Commonwealth of Pennsylvania and having an office at P.O. Box 41, Concordville, Pennsylvania 19339. 2. Defendant is a corporation having its offices located at 50 Market Street, Lemoyne, Pennsylvania 17043. 3. At all times relevant hereto, Plaintiff had issued a policy of insurance insuring a motor vehicle owned by Stephen B. Wagoner. 4. On or about June 11, 2003, a vehicle owned by Defendant and being operated on Defendant's behalf by an agent, servant or employee, struck the vehicle of Stephen B. Wagoner causing damages in the amount of SIX THOUSAND SIX HUNDRED FORTY and 51/100 ($6,640.51) DOLLARS. 5. Defendant, acting through its agents, servants or employees, agreed to reimburse Plaintiff in the amount of FIVE THOUSAND and 00/100 ($5,000.00) DOLLARS for the damages that Plaintiff was required to pay to repair its insured's vehicle. 6. To date, THREE THOUSAND SEVEN HUNDRED FIFTY and 00/100 ($3,750.00) DOLLARS has been paid to Plaintiff, but Defendant owes ONE THOUSAND TWO HUNDRED FIFTY and 00/100 ($1,250.00) DOLLARS, together with costs to the Plaintiff.
